Section 1: Revalidation of Certificate (Request rating reviews or appeals by seperate form)
Please review your last valid certificate and note any inaccuracies or changes to your boat. Please check either yes or no. If yes, please give details below or attach (upload) additional information. If you have made a change and it is properly indicated on your last valid certificate, select no. (DEFAULT answer is NO)
Have hull or appendages (keel, rudder, etc.) been replaced or altered in any way? * No Yes
